Objectives: | The CRONOS project will define, develop, and evaluate an integrated system of care delivery for chronic patients with multiple pathologies (COPD and cardiovascular conditions). It will provide an end-to-end architecture based on international standards, allowing for continuous extrahospital care. It will enable personalized and constant surveillance, thus contributing to improving the health and quality of life, and consequently reducing healthcare costs. |
